Asante Physician Partners is a group of top medical professionals working together to provide comprehensive, readily accessible care to patients in southern Oregon. At Asante Physician Partners, the Care Transformation RN will work together with a collaborative team of professionals in coordinating care for our patients' prior to and after Cardiovascular or Thoracic Surgery. This individual would be well organized, self motivated, and have a good mix of nursing skills and administrative abilities. They would work with both patients and their families in a caring and compassionate manner.

What You'll Do

Pre-operative case management of patients prior to surgery

Review the patient history, including tests, medications and previous procedures

Coordinate services for the patient

Collaborate with Providers during the patient's care, both as an outpatient and as an inpatient

Work with a team of professionals dedicated to providing high quality care to our patients

As an RN in this role you would be an important member of a team that provides patient centered care

Who You Are

A licensed RN in the state of Oregon, or able to become licensed prior to start

Well organized, efficient and self motivated

Able to proficiently use Excel and other computer programs

Compassionate, patient, and confident in your nursing abilities

What You Must Have

Licensed Registered Nurse in the State of Oregon, or able to become licensed prior to start.

Basic Life Support (BLS) certified by the American Heart Association (AHA) required upon start

3-4 years clinical experience required
