In the role of Clinical Case Manager for Cardiovascular Services you will teach and promote health education and wellness programs to the community and prospective Asante patients. You may also emphasize risk factor modification by promoting behavioral changes.

As a Clinical Case Manager for Cardiovascular Services at Asante, you will have the opportunity to:
  • Help patients return to their normal activities after having a cardiac event
  • Work with multi-disciplinary team to develop individual treatment plans for cardiac patients
  • Work in a high energy, positive environment to assist patients on their road to recovery through exercise, nutrition, stress management, smoking cessation, and other life-style behavioral changes
  • Teach education classes on blood pressure management, exercise, diet and CAD risk factor modification


  • Bachelor's degree in a nursing or health-related field or the equivalent in education and experience required (such as an Associate's in nursing and 2 additional years of experience)
  • BSN or Master's degree in a nursing and/or allied health profession, preferred
  • Minimum of two years recent RN experience in the health education field with evidence of teaching expertise required
  • CCU Experience, preferred
  • Registered Nurse (RN) licensed by Oregon State Board of Nursing
  • Basic Life Support for the Healthcare Provider (BLS) certified through the American Heart Association(AHA) required upon start
  • Advanced Cardiac Life Support (ACLS) certified through the American Heart Association required upon start
Unit Specific requirements
Cardiac Rehab:
  • Demonstrated proficiency in multisystem and psychosocial assessment and intervention, dysrhythmia identification, and application of emergency cardiac care measures required
Cardiac Education:
  • RN with 2 years experience in the last 5 years in a cardiovascular area required
Required Skills (Cardiac Rehab)
  • Ability to apply teaching/learning principles to various age groups including young and older adults
  • Ability to organize and follow through with educational programs/projects tailored to individual learners or audience
  • Demonstrated clear verbal and written communication skills
  • Ability to work in a multidisciplinary team setting with collaboration, flexibility, motivation and enthusiasm
  • Ability to reinforce and motivate desired learning behavior and act as a role model by commitment to a healthy life style
