Position Summary

Asante Physician Partners (APP) is a group of top medical professionals working together to provide comprehensive care to patients in Southern Oregon. We collaborate with other health care professionals throughout the area to offer a full array of quality care close to home, supported by Asante's full range of services and hospitals. Our team-based clinics operate on a Patient-Centered Primary Care Home model. This allows physicians to do what they do best: take care of patients while being supported by a team of specialty professionals.

The APP Care Transformation Nurse provides leadership, guidance, training, and support for Primary Care and physician practices and nurse care managers may be embedded in the clinics, hospitals, and payer setting. The Care Transformation nurse will be well versed in Population Health, team-based care, data and information management, disease and condition management and care in an integrated healthcare setting with a focus on managing the health of a designated population through integrated and coordinated care. The role will lead care teams in behavior skills, education, and care coordination to engage in improving the health and lifestyle behaviors to achieve optimum health and wellbeing of a designated population. The current ambulatory care setting is diverse and multifaceted, requiring nurses highly skilled inpatient assessment and with the ability to implement a broad range of nursing interventions in a variety of settings. With the use of analytical skills and process improvement, the Care Transformation Nurse will engage with the providers and team to enhance the care of the patient in a compassionate manner and with a commitment to excellence.

Qualifications

Education

  • Bachelor's Degree in Nursing (BSN) preferred.

Experience

  • Minimum of 3 years clinical nursing experience is required.

License/Certification

  • RN: Registered Nurse licensed by the Oregon State Board of Nursing (OSBN) required upon start.
  • BLS: Basic Life Support for the Healthcare Provider certified through the American Heart Association (AHA) required upon start.

Preferred Certifications

  • WCC: Wound Care Certified
  • WOCN: Wound, Ostomy, Continence Nurse Certification
