Position Summary

The MDS and Intake Coordinator RN is responsible for reviewing referrals to meet criteria for admitting to the Swing beds, coordinating the admission and authorization, and ensuring the timely completion of accurate assessments and interdisciplinary care plans that meet Federal and State guidelines. The MDS Coordinator identifies resident acuity and communicates needs and expectations to families and responsible parties. This role is responsible for assessments through the Resident Assessment Instrument (RAI) process, Minimum Data Set (MDS) completion and transmission. The Nurse works with the interdisciplinary multidisciplinary team to ensure care planning guidelines for each patient are followed and compliant with Medicare and quality care requirements. They ensure all plans for all residents are assigned and monitored. This role is viewed as a key financial and clinical member of the team and understands the financial concepts and implications of RUG IV, PPS, and state case mix. The MDS coordinator is responsible for setting RUG levels for each patient and ensuring the facility is getting accurate and maximum reimbursements.

Qualifications

Education

  • Bachelor's degree in Nursing, preferred.
  • Completion of MDS training program, preferred.

Experience

  • Minimum one year demonstrated experience in MDS coordination or related experience in a clinical or healthcare setting required.
  • Proficiency in MDS 3.0 required.

Licensure/Certifications

  • RN: Registered Nurse by the Oregon State Board of Nursing required upon start.
  • BLS: Basic Life Support for the Healthcare Provider certified through the American Heart Associations (AHA) required upon start.
  • RAC-CT: Resident Assessment Coordinator-Certified, preferred.
