Intake and Referral Clinician ARRMC (Behavioral Health Unit)

Support the expansion and enhancement of our behavioral health programs by collaborating on patient care as a part of an interdisciplinary care team-based approach with physicians, other providers, nurses, and health care professionals with a goal of improving health care quality. Responsible for helping shape and direct behavioral health services of the clinic in collaboration with other members of our staff. Performs individual patient assessment of needs, develops patient specific care plans, educates patient on resources available to them and conducts follow-up evaluations to determine patient outcome. Performs appropriate screenings and educational assessments as appropriate. Together with physician leadership, the LCSW will be accountable for maintaining quality integrated behavioral health programs for patients and staff. Fields and conducts patient calls as requested.
