Position Summary

The Substance Abuse Navigator is an integral team member who assists patients with substance use disorders. Introduces patients with substance use disorders to available programs and services; conducts brief assessments and schedules their appointments. Assists with access to other services, such as financial counseling, primary care, behavioral health services, social services, and residential and outpatient treatment settings.

As a Substance Abuse Navigator for Asante, you will have the opportunity to:

  • Communicate with staff daily to screen for and recruit eligible patients and provide updates to clinical team as needed.
  • Communicate and interact with patients in a culturally competent and relatable way.
  • Participate in data collection activities to support departmental programs and services.
  • Monitor and report on patient outcomes and actively work with other members of the
  • interdisciplinary team to facilitate improved delivery of cost-effective services.
  • Participate in training and development by sharing knowledge and special skills with others, such as providing
  • in-services to staff following attendance at educational events.
  • Promote desired patient outcomes by means of introducing patients with substance use disorders to available programs and services.

Qualifications

Education

  • Bachelor's of Psychology, Social Work, Health Care Administration, or related human service field - or an equivalent combination of education and/or work experience, required
  • Master of Social Work, preferred

Licensure/Certifications

  • Possession of a valid Driver's License with proof of safe driving record for the past ten years and proof of auto insurance may be required. Must have Oregon Driver's License within 30 days of moving to Oregon (if applicable).

Preferred Licensure/Certifications

  • LCSW: Licensed Clinical Social Worker
  • LMSW: Licensed Master's Social Worker
  • CDAC: Certified Alcohol Drug Counselor
