Population Health Care Management Specialist

Position Summary

The Population Health Care Management Specialist plays a vital role in the Population Health Care Management team, requiring the individual to demonstrate initiative, strong critical thinking, organizational, communication, and problem-solving skills, as well as a commitment to quality and excellence. The position involves essential functions of care coordination and collaboration with patient family, physician, and patient care team to ensure the delivery of high-quality, efficient, and cost-effective healthcare services, aiming to avoid hospitalization and emergency room visits.

Qualifications

Education

  • High School Diploma, GED or equivalent experience demonstrating the ability to effectively communicate as required for the position, required
  • Associates degree in Business, healthcare administration, or related field or equivalent education and experience may be considered, preferred

Experience

  • 3 or more years in-patient or out-patient experience in a healthcare environment with a good understanding of medical terminology, required
  • Experience with community outreach, patient advocacy and coordinating care in areas such as acute care, skilled nursing, home health or similar areas dealing with effective communication and administrative functions, required
  • Experience with Microsoft Word and Excel, required
  • Experience with EPIC, preferred
  • Knowledge of medical insurance, preferred
  • Experience as a Medical Assistant, preferred
