Performance Improvement Analyst - Population Health

Position Summary

The Population Health Performance Improvement Analyst supports the Population Health Department and Asante Health Network (AHN) by ensuring efficient operations and aiding in achieving strategic objectives. The key responsibilities of the Population Health Performance Improvement Analyst include managing AHN provider data and ensuring its accuracy; onboarding new provider groups to the Asante Health Network (AHN); ensuring the AHN website is up to date; implementing and maintaining policies and procedures for population health programs; tracking action items and coordinating program development; supporting programs like network provider relations, data analytics, payer partnerships, and internal operations; and enhancing operational efficiency, quality outcomes, and customer service to advance AHN's mission.

Position Qualifications

Required Experience

  • Minimum 7 years of experience coordinating health care operations in a health system, physician practice or health plan setting to include advanced project management, database management, provider relations, communications, marketing and related duties.
  • Strong analytical skills.
  • Knowledge of database concepts and ability to apply professional principles to effectively summarize data.

Education

  • Bachelor's Degree or an equivalent combination of education and/or experience is required.
  • Master's Degree in business, healthcare, or related field is preferred.
