Position Summary

In the role of Clinical Case Manager for the Cardiac Center, you will teach and promote health education and wellness programs to the community and prospective Asante patients. You may also emphasize risk factor modification by promoting behavioral changes.

Qualifications

Education

  • Bachelor's degree in a nursing or health-related field, preferred.

Experience

  • Minimum of 2 years progressive nursing experience in an acute care setting preferably within discharge, utilization and/or case management, or the equivalent combination of education and/or experience, is required.

Licensure/Certifications

  • RN: Registered Nurse licensed by Oregon State Board of Nursing (OSBN) required upon start.
  • BLS: Basic Life Support for the Healthcare Provider certified through the American Heart Association (AHA) required upon start.
  • CCM: Certified Case Manager, preferred.
