Salary: Typical Starting Range is $24.32 to $30.40 (based on years of relevant experience)

The Asante Physician Partners LPN provides care in an integrated healthcare system with a focus on managing the health of a designated population in an ambulatory clinic setting. The LPN in the ambulatory setting will utilize change behavior skills, patient education, and care coordination to engage with a designated population. The goal of this engagement is to improve the health outcomes for designated population, while improving lifestyle behaviors to achieve optimal health and well-being for designated population. The current ambulatory care setting is diverse and multifaceted, requiring nurses highly skilled inpatient assessment and with the ability to implement a broad range of nursing interventions in a variety of settings. Utilizing the nursing process, delivering superb health care in a compassionate manner, is committed to excellence and empowered to streamline and improve operations. They are uniquely qualified to influence organizational standards related to patient safety and care delivery in the outpatient setting.
The Advanced Wound Center has a dynamic team who are passionate about the work they do. This position will require an individual who excels as a team member, enjoys wound care, loves to learn and develop new skills and is comfortable with a physically active work environment. The Wound Ostomy Nurse provides direct and indirect care for patients with acute and chronic wounds, tubes, ostomies, burns and pressure ulcers.
This position for the Advance Wound Center is for both inpatient and outpatient settings. The clinic is open Monday-Friday and closed for most major holidays.
- Completion of a wound care certification program preferred upon hire or if eligible within a year of hire
- Completion of an ostomy care certification program preferred.
- Wound Care experience preferred
- Experience in an acute patient care environment is preferred
- Licensed Practical Nurse (LPN) licensed by Oregon State Board of Nursing.
- Basic Life Support for the Healthcare Provider (BLS) certified through the American Heart Association (AHA) is required upon start
