Asante Hospice is a dynamic interdisciplinary team, working with terminally ill patients and their families. The Asante Hospice assists patient and families with psychosocial / emotional support, life review, end of life planning as well as educating family members in the practical aspects of care required at end of life. . Each Social worker is assigned a case load of patients/families in conjunction with a Registered Nurse.

Our team is dedicated in supporting families, and each-other. While the work can be difficult at times it is also extremely rewarding. The ASANTE HOSPICE TEAM is committed to providing educational support, debriefing, and encourage creativeness aspects of the Program.

POSITION SUMMARY

The Supervisor for Hospice/Social Work is responsible for assisting the Hospice Manager in the development, monitoring and supervision of Hospice/Transitions MSW's and Hospice Spiritual Care providers/ Chaplains. In this role, the Supervisor also provides Social Worker services as needed for Hospice and Transitions clients.

This position requires flexibility and autonomy.

QUALIFICATIONS

Education

  • Master's degree in Social Work required

Experience

  • Minimum 2 years of post-master's social work practice preferably within a Hospice or palliative care environment is required
  • Knowledge of Hospice and/or palliative care, strongly preferred
  • Supervision experience preferred

Licensure/Certifications

  • ODL: Oregon Driver's License with proof of safe driving record for the past 10 years and proof of insurance required
  • LCSW: Licensed Clinical Social Worker by the State of Oregon, preferred
