Remote Licensed Clinical Social Worker APP (Care Management BHC)
Our Licensed Clinical Social Worker position represents an innovative step forward in the relationship between primary care and behavioral health in our community. The Behavioral Health Consultant and as a key member of our interdisciplinary primary care team. The Licensed Clinical Social Worker is responsible for directing and shaping the integrated behavioral health services of the clinic in collaboration with the primary care team. As an expert in Behavioral Health, this position will work closely with the primary care providers to understand the root cause of patient's psychosocial issues and implement effective treatment plans. The Licensed Clinical Social Worker supports the enhancement and expansion of our behavioral health programs.

As the Licensed Clinical Social Worker, you will have the opportunity:
Perform individual patient assessment of needs and develop patient-specific care plans, collaboratively with the primary care team. Ensuring high quality well rounded patient care.
Collaborate with providers and the care team to develop and implement structured, evidence-based behavioral health treatment as part of a coordinated treatment plan with a goal of improving quality outcomes.
Educate patients on resource available to them and conduct follow-up evaluations.
Qualifications
Education
Master's degree in Social Work.
Experience
Experience with psychosocial assessment, individual therapy, short-term evidence-based counseling, and crisis intervention.
Experience working in the medical social work or community mental health field and knowledge of Integrated Behavioral Health in the community healthcare system.
Experience working with health-related issues, substance abuse, and history of trauma.
Licensure
Behavioral Health Consultant LCSW (Licensed Clinical Social Worker)
Licensed Clinical Social Worker (LCSW) by the State of Oregon
