Asante Physician Partners (APP)is a group of top medical professionals working together to provide comprehensive care to patients in Southern Oregon. We collaborate with other health care professionals throughout the area to offer you a full array of quality care close to home, supported by Asante's full range of services and hospitals. Our team-based clinics operate on a Patient-Centered Primary Care Home model. This allows physicians to do what they do best: take care of patients while being supported by a team of specialty professionals.

With our progressive primary care team approach, we leverage the expertise from patient care, behavioral health, and pharmacy to provide comprehensive treatment plans. The Community Resource Coordinator (CRC) functions as a member of the Care Management team. The CRC is a frontline public health worker who has an understanding and connection to the community they serve. CRC utilizes communication, organizational, and problem-solving skills to provide a link between the patient, health care and social services, as well as services offered by the community in which the patient resides. The CRC communicates with all areas of the clinic to ensure that at risk patients are identified and promotes and evaluates the effective utilization of resources, ensuring the patient receives adequate follow-up, and advocating for the patient when necessary.
