POSITION SUMMARY

Asante is starting an ambulatory palliative care program that will be embedded in the oncology clinic providing services to cancer patients and, as the service develops, to include patients with end stage heart and lung disease. Join a palliative care physician in starting this new service.

Working within the framework of social work values, knowledge and method, the palliative care social worker will be an integral member of the palliative care team, providing evidence based palliative care interventions, utilizing their expert lens to provide patient care, and contribute to the palliative care team's process and wellbeing. This requires a commitment to whole person care as demonstrated by their commitment to ethics, biopsychosocial and spiritual assessment.

This position requires excellent communication skills and the ability to collaborate and work in a team environment. Exceptional candidates will have the ability to provide empathy, compassion and support to patients, families and fellow team members. In addition, it will require culturally relevant approaches to interactions and interventions.

The ambulatory palliative care team will work closely with the inpatient palliative care team and hospice, ensuring smooth transitions of care and ongoing support.

Prior experience in palliative care preferred, but not required.

QUALIFICATIONS

Education

  • Master of Social Work

Experience

  • Prior experience in palliative care preferred, but not required.

Licensure/Certifications

  • ODL: Possession of a valid Oregon Driver's License may be required with proof of safe driving record for the past 10 years and proof of insurance
  • LCSW: Licensed Clinical Social Worker, preferred
  • LMSW: Licensed Master's Social Worker, preferred
